Medical Forms:

The previous medical forms with Class 1, 2, and 3 have been replaced in 2010 with a new form that is available now (the “Annual Health and Medical Record”).

The Annual Health and Medical Record (the new form for 2010, updated again in 2011):

·       For 2011, they re-arranged the form and put it in a more logical order. Now, parts A and B are needed for all short-term events (less than 72 hours), while part C with medical exam is needed for over 72 hours, such as Lyle or High Adventure. Troop 129 will accept 2010 forms as long as the information is up to date, but prefer to have the 2011 forms on any new ones.

·       Parts A and B of the new form are to be completed annually by all BSA unit members, and cover all events under 72 hours (similar to the previous Class 1). Medical information required includes a current health history and list of medications.

·       Part C is required along with parts A and B for any event that exceeds 72 consecutive hours, such as Lyle or high adventure. Please follow the link above to the form for additional details.
